Insurance

Our office is pleased to accept most PPO insurance policies as a courtesy to our patients.  It must be clearly understood that the insurance “contract” is between you, the patient, and the insurance company.  Therefore, you are responsible for any amount not paid by the insurance company.  Although we are happy to complete insurance information forms and submit a claim on behalf of the patient, we do not accept responsibility, under any circumstance, for the outcome of the transaction. Completing insurance forms is a courtesy we extend to our patients in an effort to maximize their likelihood of obtaining insurance reimbursement. By having our office process insurance forms, the patient agrees to accept liability for those forms.  

Patients must pay the estimated co-payment (the amount not covered by the insurance company) at the time services are rendered.  This is an estimate only, and this amount may change based upon final insurance company payments. Our office does not guarantee that the patient’s insurance company will pay for any procedure therefore you are responsible for any amount not paid by the insurance company.

Please understand that our treatment plans for the dental work diagnosed are not based only on what your insurance covers. We will provide a comprehensive treatment plan based on your individual dental needs and will not let insurance coverage dictate your needs. The choice is yours on what to do or not to do.  However, it is our responsibility to inform you of any conditions we see during an exam.

We do not accept any HMO’s or Medicare.

Payment & Financing Options

For our insurance patients, all patient co-payments are due in full at time of service. We will process and file all insurance claims for patients, with payment being sent directly to our office. Please bring your insurance plan information with you on your first visit. Our staff will obtain your insurance benefits from your carrier, and we will provide you with an estimate of what your insurance will cover.  

For our clients without insurance coverage, payment in full is due at time of service. For your convenience, we offer several payment options:

  • Cash or Personal Check
  • Credit Card – We accept MasterCard, Visa and American Express.
  • Care Credit is another convenient, no initial payment, low monthly payment plan for dental treatments. Care Credit works like a line of credit. Upon approval, patients are provided a Care Credit card for their dental treatments. We are pleased to provide our clients with a care credit option of no interest for up to a year.  Use up to the maximum approved for. As payments are made against the balance, you may begin using the new amount available again. You can apply in our office or apply online
  • Lending Club Patient Solutions offers no initial payment and no payments for 3-7 weeks, affordable extended plans with low fixed rates and low monthly payments, interest free plans, and no prepayment penalties.  You can apply in our office or apply online.
